Great article from Vanity Fair about the El Faro loss - this is a must read for anybody who goes out on big water - oceans, Great Lakes, etc. It's never just one thing that leads to tragedy - it's a combination of overconfidence, equipment problems, unexpected weather, and an unwillingness to adapt to changing conditions.
